Services. This new senior community is in a beautiful and serene
location atop the bluffs of Dubuque, Iowa, overlooking the
Mississippi River.This community offers the following care options:
Residential Care Facility (RCF), Residential Care Facility (RCF)
with Memory Care, Long Term Care/ Care CenterAddress: 1160 Carmel
Dr, Dubuque, IA 52003Near by towns include: Bellevue, Cascade,
Peosta, Maquoketa, Platteville, WI, and Galena,
IL.Responsibilities:As a Speech Language Pathologist you will be
responsible to:Conduct a comprehensive assessment & evaluation
using subjective and objective measures.Develop treatment plans and
programs in accordance with physician orders.Provide quality
treatments, actively progressing goals to achieve highest quality
of life.Conduct screens and provide education to residents/clients,
families, caregivers, and staff.Coordinate seamless discharges
utilizing the full continuum of therapy and fitness
services.Provide accurate and timely documentation & billing in
electronic medical record.Demonstrate understanding and knowledge
of payer sources and regulatory requirements.Complete and document
supervisory visits per regulations.Communicate effectively with
residents/clients, families, caregivers, physicians, referral
sources, and interdisciplinary team.Qualifications:Graduate from an
accredited Speech-Language Pathology Program with a Masters Degree
in Speech-Language Pathology.Current licensure in the state of
practice as required and current 'Certificate of Clinical
Competence' from the American Speech-Language Hearing Association
or currently completing a 'Clinical Fellowship Year' with a
Certified supervising Speech Language Pathologist.Competence in
Speech Language and swallowing evaluation and
treatment.Demonstrated compatibility with PHS's mission and
operating philosophies.Demonstrated ability to read, write, speak
and understand the English language to communicate with all
