Overview: Responsible for utilizing medical knowledge to assess
patient needs for the purpose of developing a plan for assisting
patients to achieve their optimal level of health. Duties include
telephonic assessment, care planning, implementation, coordination,
monitoring, evaluation and outcomes measurement within the scope of
their license. The LPN care coordinator is a patient advocate that
promotes safety and maintains privacy and confidentiality in
accordance with policies and procedures.
This position is full-time, Monday - Friday, no weekends or nights!
This is an in-office position until successful completion of the
orientation period and then there is an option for a hybrid option
of two days working from home and three days in the office.
PM21Requirements: QUALIFICATIONS:
1) LPN with an unrestricted license/certification in the state or
in a state that has licensure reciprocation with the state of the
office location the employee is working in. 2) Successful
completion of case management/disease management orientation
program; 3) minimum of 2 years full time equivalent of direct
clinical care to the consumer 4) Excellent communication skills; 5)
Basic typing/computer knowledge with minimum keyboarding speed of
35WPM; 6) Previous case management experience helpful but not
required.
